Initial referral fee: $30. Further comprehensive referral, if required: $30. Pathology provider charges may apply separately.
Request initial pathologyComplete the comprehensive health questionnaire and provide your pathology and relevant treatment documents. If transferring care, you can include a pharmacy label clearly showing your printed name and current medication, or current clinical information from your treating clinic or doctor.
