What to have ready.
We accept transfer-of-care requests from men currently undergoing TRT. The doctor needs enough information to assess your diagnosis, current treatment and recent monitoring.
- Pre-treatment bloodwork and supporting information about your original diagnosis.
- Your current treatment details: a pharmacy dispensing label clearly showing your printed name and current medication, or current clinical information from your treating clinic or doctor. A clinic summary, treatment correspondence or similar documents can also be provided.
- Recent monitoring results, including results from the past 12 months where available.
- Current comprehensive pathology completed within the last few months.
What happens next?
Complete the comprehensive questionnaire and upload the documents you have. Our team will review the information and advise whether anything further is needed before a doctor consultation can be arranged.
Transfer requests require an individual clinical assessment. Existing treatment does not guarantee continuation of the same prescription or protocol.
